What is the difference between forum moderator and forum ninja?

Forum Ninjas and Forum Moderators, while they both have forum moderation power, have slightly different roles (aside from the fact that Ninjas make up a Sr. Mod team). Forum Mods are centered entirely around moderating the forums and helping out there, while Forum Ninjas have the additional task of responding to punishment appeals - the only staff team capable of such. Other slight differences constitute the main difference, as well.
What is the difference between Ultra and Hero and how do I get Ultra?
