What is the glitch?
This bug allows dwarfs to damage or possibly kill other dwarfs using the enchantment flame, on a bow.

What causes the glitch?:
The flame enchantment if used on a bow.

How can I re-create the glitch?:
You need to enchant a bow, with a flame enchantment. Then any dwarf will be able to shoot flame arrows at other dwarfs and damage dwarfs by setting them on fire. If the dwarf hit had low health, this would allow for dwarfs to kill another dwarf accidentally or intentionally.

How common is the glitch?:
This bug is common, as any dwarf will be able to easily utilize it and damage other dwarfs

How many times have you seen the glitch?:
I have only seen it be used accidentally, but I wouldn't be surprised if dwarfs started to intentionally hurt other players.
